UK/EU legislation requires all candidates to provide appropriate evidence for applications towards all employment. In line with UKBA guidance on the prevention of illegal working, we will need to verify and take a copy of your original ID documentation as evidence of your right to work in the UK/EU.

We ask this in order to find out your needs in terms of reasonable adjustments to access our recruitment service, and to perform the job or position sought.

*Criminal record disclosure:

We are asking you to complete this form in relation to job roles which are not exempt from the Rehabilitation of Offenders Act 1974. For this reason, you are only required to disclose information about unspent convictions. You are not required to disclose spent convictions on this form.

Additionally, you are not required to declare any information about ‘protected’ offences – (offences to which the filtering rules apply). If you are unsure as to whether a conviction is unspent/spent or protected (filtered) please see the additional guidance or you can contact organisations such as NARCO or Unlock for further information.

If you wish to be put forward for/if any role is identified which may be suitable for you but which is exempt from the Rehabilitation of Offenders Act 1974, meaning that you are required to disclose spent convictions, we will ask you to complete an additional criminal disclosure form. You are not required to complete that additional form if you do not wish to be put forward for that type of work.

I hereby confirm that you have my authority to seek references from pervious employers and upon successful placement by Michael Sharma Group, I confirm that you are able to seek references from my current employer.

I confirm that to the best of my knowledge in the information contained in my CV is correct and true. I have omitted no facts that could affect my future employment and accept responsibility if it is found that I have deliberately given false or misleading information.

I will inform Michael Sharma Group immediately of any circumstances that may affect my work, such as changes to health, subsequent pending prosecutions or convictions, which may arise whilst I am registered for permanent, contract or interim work.

I acknowledge and agree that should Michael Sharma Group introduce me to a company and my application is unsuccessful I will not approach the company with a direct approach for at least 12 months. Instead, should I wish to be reconsidered I will contact Michael Sharma Group to request that they resubmit / introduce my details.

I agree that the above information given on my application may be used for registration purposes under applicable data protection legislation (including but not limited to the General Data Protection Regulation (Regulation (EU) 2016/679) (“GDPR”).

I confirm that upon successful job offer and verbal acceptance, I authorise Michael Sharma Group to transfer my data to their employer client so that a written offer can be made. I also authorise Michael Sharma Group and its nominated agent(s) to approach credit referencing agencies, educational establishments and government agencies where appropriate to verify the information that I have given.
